CERT Message Forms

Communications is one of the biggest challenges in disaster response. This is especially true for citizen disaster response. Citizen volunteers often have little training on proper radio use. The attached templates were designed to help CERT Search & Rescue (SAR) teams communicate efficiently and effectively with their CERT Command Post.

They were developed to support our primary mission - search and rescue of our neighbors following a major disaster - as well as to provide our city EOC (Emergency Operations Center) the information they seek.

The Request Message Form - To be used when the SAR team needs something like water, victim transport, team transport, replacement personnel or a tree moved out of their way.

The Hazard Message Form - To be used when the SAR Team sees a hazardous situation or one that requires a response. Examples include fire, major gas leak, or a tree blocking a road they are not using.

The Status Form - To be used by a SAR Team every 30 minutes to report position, casualties encountered, ground covered, etc.
